Oswego Health Announces Leadership Promotions Within Oswego Health Home Care

Oswego Health is proud to announce two key leadership promotions within Oswego Health Home Care, further demonstrating the organization’s commitment to growing and developing talent from within.
Jeff Hopp, BSN, RN, CCM, has been promoted to Administrator of Oswego Health Home Care. In his new role, Hopp will oversee the agency’s operations and ensuring the delivery of safe, effective, and compassionate home health services. As Administrator, he will be responsible for guiding agency strategy, quality initiatives, and the ongoing advancement of patient-centered care.
Hopp joined Oswego Health Home Care in 2015 as a Registered Nurse Case Manager and quickly advanced through several leadership roles, including Clinical Coordinator, Team Manager, and, most recently, Director of Patient Services. With a strong clinical and administrative background, Hopp successfully guided the agency through transformative changes, including an ownership transition, electronic medical record implementation, and pandemic-related challenges. Before joining Oswego Health, he worked in the Medical Intensive Care Unit at St. Joseph’s Hospital.
His education includes a Nursing degree from St. Joseph’s College of Nursing, a Bachelor of Science in Nursing from Capella University, and a Population Care Coordination Certificate from Duke University School of Nursing. Among his many academic honors are the Sister Ann Francis Award for enthusiasm in learning and repeated Dean’s List recognition.
Jennifer Nickolas, BSN, RN, COS-C, has been promoted to Director of Patient Services for Oswego Health Home Care. In this role, she will oversee day-to-day clinical operations, directing staff, and the coordination of quality care for patients receiving services in their homes. She will play a key role in ensuring compliance with state and federal guidelines while fostering a culture of excellence among the agency’s staff.
Nickolas began her career at Oswego Health Home Care in 2012 as an RN Case Manager and has since held several advancing positions, including OASIS Auditor, Clinical Coordinator, Team Manager, and Director of Patient Services on an interim basis. She brings more than 25 years of diverse nursing experience, including leadership roles at The Manor at Seneca Hill and clinical positions in dialysis and long-term care.
Nickolas earned her Associate in Nursing from Cayuga Community College and a Bachelor of Science in Nursing from Chamberlain University. She is a Certified OASIS Specialist–Clinical and brings extensive skills in patient assessment, wound care, medication management, and quality improvement.
Together, Hopp and Nickolas have worked side by side for years, guiding the agency through pivotal industry and organizational changes—including the pandemic, ownership transitions, and evolving care models—making them a proven and effective leadership team.
