The Assertive Community Treatment Team, or ACT Team is a mobile, multi-disciplinary, mental health team, which offers various support services and delivers comprehensive, person-centered treatment and rehabilitation to individuals over the age of 18 with severe and persistent mental illness.
ACT Team services include help and support 24-hours a day, 7 days a week.
The ACT Team:
- As a multidisciplinary team, rather than refer individuals to other programs and services, the ACT Team provides the treatment and services needed. Services include housing and vocational support, family education, counseling, psychiatry services, physical health monitoring, medication administration and substance abuse support.
- Works with individuals in their homes, neighborhoods, workplaces, and other places where patients feel most comfortable receiving services.
- Works closely with each patient to develop an individualized plan that is followed by all team members in an effort to provide the most comprehensive care to improve patient outcomes.
