Case Management

Oswego Hospital's case managers are available to every patient, regardless of the reason for a hospital stay. Before leaving the hospital, our registered nurses and social worker case managers can assist you with arranging appropriate community and home care services such as:

Case managers also assist with:

  • Advocating for you as needed in the healthcare system
  • Questions regarding your Medicare/Medicaid rights and the discharge appeal process
  • Information regarding community resources available to you and, when appropriate, referral information for housing, support groups, crisis counseling and mental health programs
  • Advance directives such as a Health Care Proxy form
  • Reviews to your commercial/HMO insurance provider regarding your admission, length of stay and use of hospital resources
  • Referring you to our quality department if you have concerns with your care
  • Arranging for your family to meet with the Ethics Committee to review your care if there are any concerns or disagreements with the plan of care
