The Short-term Rehabilitation program at the Manor has been designed for the person who has been discharged from the hospital but requires additional short-term skilled nursing or rehabilitation before returning home. Individuals needing special attention following surgery, accident, stroke, hip or knee replacement, or needing rehabilitation after a fall can be cared for in the comfort of our transitional care site.
Short-term rehab can last anywhere from one week or longer, depending on the individual’s needs and goals. A team of medical professionals meets with each patient to create a personalized plan of care to help meet the goals of regaining function and returning home. Family members are an important part of the team and are encouraged to participate in the rehab program.
Full time physical and occupational therapists along with speech pathologists work to help restore physical and cognitive ability quickly. Equipment is specially designed for short-term rehabilitation needs, with additional equipment for stroke survivors, such as the facility’s new litegait.
The short-term rehab program addresses not only the physical but also the emotional, psychological, social, and spiritual needs of patients and families. Patients are invited to participate in daily activities, exercise programs, religious services and social events. Three great meals plus a choice of snacks and drinks are available, and our dietitian can meet special dietary needs.
Patients receive 24-hour access to pain management and nursing care. An interdisciplinary team of nurses, nurse practitioner, physicians, social workers, therapists and a registered dietitian provides care. On-site medical experts stay in communication with each patient’s primary care physician to ensure a smooth transition and continuity of care.
Accommodations include semi-private rooms and some private rooms. Every patient has their own window and their own side of the room. Televisions and phones in every rehabilitation room offer the comforts of home. Beautiful lounges, patios and lobby areas are available, and visitors are welcome any time. A luxurious spa room offers a whirlpool tub and shower. On-site beauty and pampering services include a full service salon and massage therapist with reasonable prices to keep these “extras” affordable.

From the beginning of your stay at Manor at Seneca Hill, our team works with you and your family to create a discharge plan. Discharge planning staff targets each individual’s needs according to the individual’s goals and environment. Departures are well planned, and patients and families are well prepared for going home, or to another level of care if that is your goal.
