Oswego Hospital’s Home Aide Service provides safe, reliable and experienced home care for you and your loved ones. The Home Aide Service for Oswego County is a community service provided by the Oswego Hospital.

Our Home Aide service understands the importance of having the option to have home care services provided in the comfort of familiar surroundings. Care in the home is also cost effective and can delay placement in a skilled nursing facility.

Our home care staff cares for those who are elderly, disabled, chronically ill, or those recuperating from an illness. All our caregivers are certified by the New York State Department of Health as Home Health Aides and  Personal Care Aide Paraprofessionals who can provide:

Personal care
Assistance with the activities of daily living
Light housekeeping
Meal preparation
Essential errands

Our Trained Homemakers can provide:
Light housekeeping
Essential errands

After an initial assessment by a registered nurse, a plan of care is developed, that has been based upon physician orders, as well as the diagnosis, prognosis, family situation and home environment.

 Monitoring care is a registered community nurse coordinator, who periodically visits the home to review the care provided, and to review and revise the care plan. They are also available to discuss any needs or concerns with clients.
 
The Home Aide Program is fully accredited by the Joint Commission.
 
Our home health care staff work for an hourly rate and some third party payments such as Medicaid, Medicare or health insurance policies may be accepted.
